Here are some survey forms you may find useful.
Survey Paper
Customized BCCS Survey Paper, based on the templates by Derek Bristol.
Run some plain paper through your printer first to be sure you know how to load the pre-punched forms.
When printing with Adobe Reader, I had to select "Actual Size" (100% scaling) and Landscape orientation. If you use the double-sized forms, set to "flip on short edge".
Laid out to print on RightInTheRain LL8511 Paper for single-sided printers.
